请问申请怀孕和育儿津贴时雇主的工作记录证明怎么开？

老婆打算申请怀孕和育儿津贴，但是材料之一是雇主开具的工作记录证明，可是老板不知道如何开，请知道的朋友告诉一下我。在哪里下载相关表格？谢谢！

this is ROE form from HRSDC, or you could order one from HRSDC

Alberta: call 1 800 561-3992 or complete the ROE order form and fax to 1 800 829-6891

. }* t: P% M$ \# k5 pyour business number employer's number issued by the Canada Revenue Agency (CRA),
7 D( |3 ~+ ~5 I# @$ Ethe name, telephone number and address of your business.
If you do not have such a number, you will have to request it from CRA before you place an order.

这是你老板的责任呀？实在不行你让他电话问EI部门吧。
只要有人再他那里工作过并quit过工作，他们就应该开roe给的。无论什么原因她们都要开的。
