Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
) ?8 S7 K5 W7 X/ MTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
, m' R' S7 y' Y+ ]2 TPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

/ G# K! x: U$ j4 y1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
) X; Y0 {  a4 Y2 Z9 v% h6 \. e+ P3. Each child is limited to 1 to 3 programs per organization
9 r4 _$ m, c( b* W4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
! b* B! J/ E* Y! `5 y; c& m6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

5 p: K+ |; \5 P5 Y+ }We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
