Target:        Low income families with children/youth aged 3-17 residing in Edmonton
7 K7 M) l& u# D/ b0 M; nDate        May 14, 2005 (Saturday)
0 `3 ?5 p7 R) s9 N; Y7 Q& T2 f5 RTime        9:30am – 11:30am
9 h* k5 K+ y: F" vPlace        ASSIST Community Services Centre (9653-105A Ave)
" p3 o; w8 k9 v7 H+ p  bPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

+ @' b9 g. u& }& g) C- P8 XRemarks:
/ M* z. @' V! |! a9 G4 K1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4 r+ Z6 t3 e0 ^7 O* {, `3 }2 W4 ^4. Information on Child’s swimming level when registering for swimming classes
/ e; c0 f) E$ @/ M: S- w7 `5. Children are not required to be present at the Registration Day
! C, n) {2 }( X4 M, Z2 _/ w6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

8 `( C* U. o- Z, v. z; v) JWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
