Primary purpose: The Social Worker works in partnership with settlement workers or . g/ B3 F, x" ]" B* S5 d- A
community cultural brokers from New Canadian Health Network partner organizations to * H: G/ [( u5 l
provide supplemental support to newcomers who have complex needs. " ^9 n/ k/ t5 X; H* b& E9 T7 \
# j/ ~! o6 M# i2 { Nature of Work: The Social Worker will report to the manager of mental health services.
- y' Y6 K; v- t u0 s/ xThe position builds working relationships with New Canadian Health Network partners’ staff, . O7 e& N$ }- e" d4 ^
newcomers accessing these services and other institutions and service providers. The Social . O7 Y2 X s; Q C& }, l% W
Worker will assist in case planning, conferencing and providing support and advocacy as
) N) f9 p5 p$ rneeded. As required, the Social Worker will assist in accessing community supports for - Y; N% P. f' A/ u+ H F
newcomers and their families. The Social Worker provides holistic and flexible initial ; o" t5 I" x2 w U6 O6 } U6 t. y
assessment of life circumstances, needs, goals and aspirations and adheres to the client-6 d4 i: g l; H) b1 C3 i
centered delivery model.

Provide intake, needs assessment, referral, case plans and action plans to support ! m5 |2 G, A4 W

Work in partnership with settlement workers or cultural brokers to provide supplemental

Qualification

. S& Q8 C& Z5 F) O W% s Relevant undergraduate degree or diploma, or equivalent combination of education and 1 ~& q4 ^7 S) a& j4 K0 v
experience in Social Work or related human service fields. ( Q' B% x2 b& x8 n; `% V6 k3 A
Extensive knowledge of community resources, including in depth understanding of family 8 I9 c& _. U0 p/ m/ }7 _! u" ]
support needs
, ?1 Y9 }. V B# y3 N) Y! B" U6 m Experience/facility with MS Office computer applications
; h7 U. c+ x, P$ B. C Proficiency in both official languages is an asset, as is fluency in other languages
