Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs.

/ n6 A" B7 u- X% b) Q Nature of Work: The Social Worker will report to the manager of mental health services.
9 k; B' v" [& y8 W; ~The position builds working relationships with New Canadian Health Network partners’ staff, ! }. D) q7 h0 N3 {3 Q, p. b4 E
newcomers accessing these services and other institutions and service providers. The Social ; T7 J/ h0 [0 c3 |
Worker will assist in case planning, conferencing and providing support and advocacy as
* n9 f2 t. U8 k0 Jneeded. As required, the Social Worker will assist in accessing community supports for
5 a7 t; G" q) Y5 z& X$ fnewcomers and their families. The Social Worker provides holistic and flexible initial 4 l* _ d9 p9 X) W
assessment of life circumstances, needs, goals and aspirations and adheres to the client-6 K4 [/ T! X- {9 A
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support 1 F, L8 \; g7 I# e# ?: P

Work in partnership with settlement workers or cultural brokers to provide supplemental 6 S3 W( X2 h9 Z- ]: m

Relevant undergraduate degree or diploma, or equivalent combination of education and . J% y/ R- Q* _
experience in Social Work or related human service fields.

Experience/facility with MS Office computer applications - v4 E1 @6 Y* F. p" v
Proficiency in both official languages is an asset, as is fluency in other languages
* ?+ W* t0 |% L$ ` Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 1 a: ^# ~/ @! s d! s1 U0 ~9 f% l
