E" A$ |The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

& H5 U  ^6 O9 R, S+ SThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

& [* Y" d& [% v•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  
+ ^9 O- G6 g' o: a4 F) [# Q•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
. y# K6 ]0 v! C) }6 r+ b. n! y8 B+ @•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.
8 Z* f! u% R( |6 h! X•        Ensure the maintenance of accurate records in line with practice policy.
+ W/ `6 ~, X' e# w( L•        To assist in developing and implementing patient self-management programs.
3 }0 i6 r, E) y' w# u  V•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

•        The primary care nurse should have excellent written and verbal communication skills.
•        He/she should be able to work in a team while respecting the views/opinions of others.
•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.
' j7 F  S% E$ ~, Q•        Requires the ability to work independently and prioritize workload.
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
•        To participate in the Network’s education program.

SELECTION REQUIREMENTS:
9 U7 }3 T: U: i+ }1 g+ m
7 v! v6 E1 \. O0 a; L2 dQualifications/Experience
; \: P  o5 Z* p/ V•        Baccalaureate degree in Nursing preferred
•        Preference given to those with at least 5 years nursing experience in primary care setting.
+ q% P7 x3 L4 o7 Y0 _# v•        Current CARNA Registration
•        Current CPR – Healthcare Provider
3 X: l3 a/ Z, Y; L( e6 b! M•        Must have own vehicle and valid driver’s license
