$ z# P" {5 G. ~8 lThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
! ^3 c/ d  k1 n7 v- S6 {, n  a, M( r•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.# }8 R! K- d# r& }! s  i9 F2 R$ e0 f
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.2 N! Z' F# a" G: O, A4 n
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.) a* p/ e" ?  l& \
•        Educator, communicator, advocate, coordination/navigation, supporter* o% n) _. A, K) P) i# }

Clinical Role

% v/ A3 C3 ^* ~5 L/ g1 D•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  
: ]: P# q, T6 z& Z$ _6 }# z, L' }* D•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. & K! l% j7 I9 l8 Q% R
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.
  u# V, k1 p% Z- k•        Ensure the maintenance of accurate records in line with practice policy.0 r# t3 B* q) Y7 h( r
•        To assist in developing and implementing patient self-management programs.
6 e4 w! e2 U  i7 j& u, q•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.. g0 c7 Q8 H, ]* w$ `. A2 R! ^; \9 T
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills." x9 {" ~' |, ]. ~
•        Requires the ability to work independently and prioritize workload.
) g9 o1 w3 h5 e+ y8 P•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.3 [# ?2 \, W% P' n, h
•        To participate in the Network’s education program.+ U' S- B/ g' L) A7 _' H

SELECTION REQUIREMENTS:; F2 R. F; h2 g5 G) k! Y

( ^$ ]/ M& M$ p1 j) u# G( U6 IQualifications/Experience
& }  Q1 ?+ v* m- @•        Baccalaureate degree in Nursing preferred
% J9 o) ?  g+ [7 w* ~% ?: D7 s•        Preference given to those with at least 5 years nursing experience in primary care setting.
! B" j8 {: h& ]) r  m, o0 C$ c4 H•        Current CARNA Registration
* ?' O/ y" \; g+ d! w•        Current CPR – Healthcare Provider
8 S$ I1 i6 T+ q. Z7 k6 S•        Must have own vehicle and valid driver’s license) N2 }% P- R- u$ _) d( k
•        Knowledge of, and ability to use, electronic medical records
