The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.+ p. M) P( t, L
: [7 f6 C& f# a( E- ^+ BThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.! W$ l `! M* h+ @

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• To assist in the implementation of strategies for improving the quality and provision of care for the practice population.& T* b \1 i# v, M: G

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. : K/ L( m: p- W
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. , o9 i3 G, C8 \
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To assist in developing and implementing patient self-management programs./ X3 j& g6 |% B: }) i; @/ r
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

• Baccalaureate degree in Nursing preferred
3 p* V# ?+ y$ u1 j" W7 b- g• Preference given to those with at least 5 years nursing experience in primary care setting.* P! o4 W! u" f7 }. K4 \) e" ?' s. J
• Current CARNA Registration
" D) Y( D, {( V+ S( a. A4 X# s( j• Current CPR – Healthcare Provider/ P3 ^1 H9 X; \, P
• Must have own vehicle and valid driver’s license
