When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic . G, R+ m! S9 @- G* g2 [condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and 4 J! g) U( j. Z: X) D' C+ v* ]& \• there have been no new symptoms or change in symptoms; and/or% O4 m; }/ N' P; j/ |+ C
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.4 b; D9 q) s9 O4 q
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
