When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic* P9 k, S* {6 W4 f
condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and 8 r* |' }2 E; g/ n• there have been no new symptoms or change in symptoms; and/or3 v5 l8 T2 z. a( v" G1 _+ t$ e
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.) S% @% O1 r4 \3 \" |9 m5 w8 }/ ~, N
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
