1.Print the Leisure Access Program Application Form.The form is also available:
* Q9 ]0 C& a  K1 o•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
•For individuals under custodial care, please also submit the address verification form.
