Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)

! i( r: n$ {. s. a* a% k2 S9 hPlace        ASSIST Community Services Centre (9653-105A Ave)
) t' d, t* U5 t2 DPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
: d8 i; f; g* x1 h' PFees        Ranges from $10 to $35 per week

0 P6 t. n3 Y. d1 q  _( J. g1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
6 e5 o( H8 U2 R% z' s2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
% K# k6 d  T  o  u8 z0 h- p2 a4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
! `8 I4 O* u! O8 H$ S6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications
. a! a, Q. O! s' ^- y: W7 O8 y5 D
% H* g) K$ W. a/ a6 bWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
