Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
( F% R: x" R' q3 |6 fPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
! w; a" r5 k- x7 y1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
9 u2 F& m: k  q8 ?# H2 `2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
' j; O3 L3 p* o3. Each child is limited to 1 to 3 programs per organization
- }" j& f& ~" P- I; J1 m4 ]+ R# k4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day

+ b; i, c* A& r2 R8 fWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
