Primary purpose: The Social Worker works in partnership with settlement workers or
! R" t: e# ?: Y% Q! m2 Qcommunity cultural brokers from New Canadian Health Network partner organizations to ' y, I% e/ \. } ?- T8 G
provide supplemental support to newcomers who have complex needs.

$ \! s, J8 [! L0 r$ u9 T; R Nature of Work: The Social Worker will report to the manager of mental health services.
, L3 W) ], {1 e Z/ T8 FThe position builds working relationships with New Canadian Health Network partners’ staff, . B% @! D9 K2 x' \
newcomers accessing these services and other institutions and service providers. The Social # l$ U: P! R {9 G2 h2 J
Worker will assist in case planning, conferencing and providing support and advocacy as 5 ]9 [( ~5 ?- m, c$ ~& @8 v* a
needed. As required, the Social Worker will assist in accessing community supports for
/ ]( f2 }" d0 Q3 x% i8 J; N8 f' jnewcomers and their families. The Social Worker provides holistic and flexible initial 4 K% I( z: m1 m$ @5 D8 W) G
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental # I9 n+ z x7 ~3 p
case plan support for clients who have complex needs. ) x2 @9 R) t! V1 k6 a
Advocate on newcomers’ behalf to programs, institutions and service providers. ! u1 s: M6 Z- e6 s6 l# z
Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Relevant undergraduate degree or diploma, or equivalent combination of education and & ^- U' ^& e8 c* r0 p8 r) f% B
experience in Social Work or related human service fields.
8 G' n% T' x9 O2 ]- ?( L Extensive knowledge of community resources, including in depth understanding of family 3 V, |" W: P! o2 p. l
support needs
; d: j1 _- _* ?' P9 { Experience/facility with MS Office computer applications
! }- K' K8 z7 O7 U) Q Proficiency in both official languages is an asset, as is fluency in other languages ( X- P. y7 \7 h
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate

Experience: Minimum of 3 years experience working in a related field
