name，email，telephone number，是否UA student或staff， alumni（不是学生需要交membership fee，好像是$12）， onecard number？

男子三对三排球，报名时间9月10日，需至少3名male player，报名费$20每队。
比赛时间9月20-10月20日，每周一和周三晚上，共四周。

Levels:
9 s2 `. f0 p2 f- TRecreational:  These teams are composed of players with no prior competitive volleyball experience.
' D8 v  ?% x3 b7 }' _Semi-Competitive:  These teams will have a mixture of skill levels on their team.  It is intended for teams composed of  a mixture of skill levels or with players who play pick-up on a regular basis or who may have played in high school.
6 N% f9 O4 X3 i9 S- X! X* WCompetitive:  For teams with players who played on their high school squad or who still currently play on a team (ie. City league, etc.)

4 B$ u) s+ g+ ]8 \! S$ y男女混合排球，报名时间10月5日，需至少男女各3人，报名费$70每队。
