The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population." I; y4 y E4 r7 x# c0 H/ {
+ i- Y4 ~: } g, V& `The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.9 W7 H6 Z5 a* l, \

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

( w, J! r- n' W+ ?• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Clinical Role

; p3 ]7 {7 a, k/ w. t% g• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines., s- i- R6 ]/ P7 f7 e# L7 t! `
• Ensure the maintenance of accurate records in line with practice policy.8 A' _ W9 x. c+ P. l9 k3 D8 d
• To assist in developing and implementing patient self-management programs.
( f* C; L. s' A, [, X, u• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

Qualifications/Experience
- n5 u3 D) Q% [6 ^• Baccalaureate degree in Nursing preferred
. ^4 P6 _7 k! w0 M# [% G1 J• Preference given to those with at least 5 years nursing experience in primary care setting.
2 X# c6 H$ U8 R• Current CARNA Registration
6 S# Q0 u7 V/ u( \5 Y! ~• Current CPR – Healthcare Provider7 ]: t, k) V5 N
• Must have own vehicle and valid driver’s license
8 w" z G& ~# O6 F0 k/ ?• Knowledge of, and ability to use, electronic medical records
