! t% f2 j" _6 k: bThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.. K, j+ r0 O) D' a) Y0 Y

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Clinical Role

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. - S! d9 V+ w1 M5 a' B. |& Y+ ]* X
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. V. t3 v$ G" X8 C- I" Q
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.! A8 m$ \, u' n1 k
• Ensure the maintenance of accurate records in line with practice policy.# C4 n( E# V8 _
• To assist in developing and implementing patient self-management programs.; s7 N* s: n9 _
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

• To offer training support for other team members and assist in their professional development.

Qualifications/Experience
) g7 C5 X( d8 E• Baccalaureate degree in Nursing preferred3 p( s' i# N% n( J; {
• Preference given to those with at least 5 years nursing experience in primary care setting.5 D3 g; Q& q6 O$ y3 q
• Current CARNA Registration
2 l1 S5 ?1 g; H) f• Current CPR – Healthcare Provider3 C. t" d* W1 P
• Must have own vehicle and valid driver’s license, y" ^8 k: v! o7 B, ]
• Knowledge of, and ability to use, electronic medical records
