The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.& z: v* Z) n- U! Q6 I5 R. B+ B
4 A$ g! p; B4 M- u+ OThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

Clinical Role

/ o- W5 F+ O( U• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.
3 N8 g C- }' S8 W4 h• To develop long-term, trusting relationships with both patients and peers in providing primary care services.
$ I# F. |3 ^4 _: j! m• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To assist in developing and implementing patient self-management programs.2 c* Y# q' G& v7 c
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used./ j2 q S* |. a
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

• Baccalaureate degree in Nursing preferred
2 h6 X2 X6 _; L! v& k• Preference given to those with at least 5 years nursing experience in primary care setting.. ~* f2 U J. P/ f
• Current CARNA Registration
0 Q3 T9 I9 J; U8 B2 n9 J8 N• Current CPR – Healthcare Provider
# I! W. z! N- i/ V* ~• Must have own vehicle and valid driver’s license% b+ G' J4 @$ l8 t
