1.Print the Leisure Access Program Application Form.The form is also available:

# i0 @. t. o! Z6 X8 h0 [ Monday-Friday except holidays, 8:30am-4:30pm.
! m0 X8 t, u+ K9 \2 }8 q. q/ \•By calling 780-496-4918 to have one mailed to you.
! F, X' t0 r( R# S& O7 v•For individuals under custodial care, please also submit the address verification form.
