1.Print the Leisure Access Program Application Form.The form is also available:
•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.

) [6 f% g  K. {0 T( x•For individuals under custodial care, please also submit the address verification form.
