Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
8 s* Y, N, {5 r0 K  `Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
       
Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
9 _$ m' k/ I5 {  P9 A& G2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
8 k3 S9 G. h5 b( c  C0 L7 G4. Information on Child’s swimming level when registering for swimming classes
) f9 O& a, O2 k5 p: l2 a5. Children are not required to be present at the Registration Day

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
