Place        ASSIST Community Services Centre (9653-105A Ave)
. b. M( i! L' J% H7 \5 ^) jPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
  G$ |4 a0 N/ Q$ a& I+ cFees        Ranges from $10 to $35 per week

1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
; O$ q. z) O% d/ j0 k, ?$ p: B3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day

Fee Reduction Pass Applications
) Y, s3 z* M" G# x6 ^5 w
% j; x2 m% E# w% d* H9 `% XWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
