! u( p2 E9 [+ s9 V2 N* ~3 Z6 HAssists in the planning, organization and implementation of meaningful programs for the residents of the Extended Care Centre in collaboration with the Activation Manager. . r4 O! c, G" s% J% {) F
Works co-operatively with the Activation Manager, facility volunteers, families and other staff members.
; B9 n- c: y+ `) `4 BMaintains appropriate resident and department records including assessments, evaluations of planned programs detailing anticipated individual participation.
[' X8 d, i7 R6 l% G1 GMaintains effective communication and good human relations with residents, families, volunteers and staff of other departments.
( B! D+ k* Q! a% E# qIs knowledgeable of the Resident Bill of Rights/Governing Principles and endeavors to promote it. & W& U" I: j% i1 X
Participates in team conferences to plan, develop and assess individual resident care plans.* G' S" Z& k3 a$ ]
Qualifications
! K+ J1 b6 O* @2 ?1 g0 u! V1 B; K% @Must have a community college recreation certificate or diploma or equivalent, as approved by the Activation Manager. Experience working with seniors, particularly in a long term care setting. 9 z5 X- t& @5 n: I6 [0 \
Must be able to communicate in written and verbal form and maintain an acceptable standard of behaviour and appearance.
( `, z7 X. m3 B" QComputer skills an asset.
