4 A& z+ t, q8 V$ d9 L Primary purpose: The Social Worker works in partnership with settlement workers or
5 W- }$ r5 M/ y! Ocommunity cultural brokers from New Canadian Health Network partner organizations to

+ Z) l- S$ K: s P4 }+ g- Z7 Q Nature of Work: The Social Worker will report to the manager of mental health services.
7 T3 \) S$ I8 l8 |( H! hThe position builds working relationships with New Canadian Health Network partners’ staff,
4 S5 u6 o, n7 \3 w! Tnewcomers accessing these services and other institutions and service providers. The Social 0 g1 l2 }) o( o( X% ^( ]
Worker will assist in case planning, conferencing and providing support and advocacy as
; z6 ~3 w( l+ H& B6 C1 e4 ^; Sneeded. As required, the Social Worker will assist in accessing community supports for * @0 S+ u; d: Q; n* P( Z
newcomers and their families. The Social Worker provides holistic and flexible initial
L: g @7 \9 o% O4 O f# o6 W4 Q2 n' e Lassessment of life circumstances, needs, goals and aspirations and adheres to the client-" t( t# ?" J, l- |+ T o" n
centered delivery model.

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support ) E" M- z$ J% L0 f
newcomers in settling and fully participating in community.
' z5 }/ X5 H+ I/ V6 N& Q Work in partnership with settlement workers or cultural brokers to provide supplemental
0 Z8 w8 `& Z. ]5 [case plan support for clients who have complex needs. ( |( W4 K O2 w) O' x G
Advocate on newcomers’ behalf to programs, institutions and service providers. ) @" {4 k9 z- \' p% R
Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Qualification
# T: [2 U' k& O* M' U
- {* i6 A6 |6 x; a Relevant undergraduate degree or diploma, or equivalent combination of education and
" {1 j. C* n" E& _& Zexperience in Social Work or related human service fields.
3 k& Y8 T% `' @' C3 q" W5 X6 y Extensive knowledge of community resources, including in depth understanding of family
0 a G6 z3 Q% t0 zsupport needs 7 `3 J+ t7 h$ Y' F! C7 ?
Experience/facility with MS Office computer applications
6 p5 v& `7 c+ _; F) A1 J Proficiency in both official languages is an asset, as is fluency in other languages
3 ]2 S; e `; v3 M Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate ! @; E3 ~$ {1 O, E
insurance coverage for work 3 X9 W" O- d! r$ w: T$ J; I

Experience: Minimum of 3 years experience working in a related field
