Primary purpose: The Social Worker works in partnership with settlement workers or
) E' C, {' K5 s7 [. F) dcommunity cultural brokers from New Canadian Health Network partner organizations to

Nature of Work: The Social Worker will report to the manager of mental health services.
6 W! l4 D' m- t6 ^8 u& }$ i7 M8 |& iThe position builds working relationships with New Canadian Health Network partners’ staff,
# `6 Z" |! _. }. c. y. k8 Mnewcomers accessing these services and other institutions and service providers. The Social + Q: Z' n# S" N% j2 h
Worker will assist in case planning, conferencing and providing support and advocacy as
5 f; [* B' q$ k' `needed. As required, the Social Worker will assist in accessing community supports for # p2 E$ I* I& M
newcomers and their families. The Social Worker provides holistic and flexible initial - z; k Y# M$ e6 c/ i& O1 m
assessment of life circumstances, needs, goals and aspirations and adheres to the client-
* V3 h4 B6 }9 W% r, ~3 r3 z$ l- X. j4 |centered delivery model. 3 ], Y, N$ T* J1 |7 ^4 w% Q5 P% e2 B

Providing holistic information or advocacy services for immigrants and refugees served. 8 f- [- n3 @7 z. f; q: f/ b6 X
Developing of social networks and community supports for newcomer families.
, B- j. ?. ^% J Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental 1 m9 H3 l) ^5 B1 O
case plan support for clients who have complex needs.

Qualification
2 X" o2 m/ T2 d# c* G9 h; Z: _; H) ] % B; j, v9 u+ D1 Y
Relevant undergraduate degree or diploma, or equivalent combination of education and
; q* W6 |; h( F; n4 C+ texperience in Social Work or related human service fields. 8 ?' y2 n" d [" B8 y! v3 M, F
Extensive knowledge of community resources, including in depth understanding of family
+ v5 a) }6 o5 C, Nsupport needs
j: V7 R- [- x+ Q Experience/facility with MS Office computer applications
+ \( @! Z+ o, ]2 L" e" |0 B Proficiency in both official languages is an asset, as is fluency in other languages
9 |6 w3 M, J- T. K& j, R/ O! X Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
