! e* l6 Z3 G+ H h& t6 P7 L( JThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.
* ^' A% }( a: E• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
* V, ?, E0 `/ B9 d8 d• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. , h2 f* [3 q1 q9 M8 y

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

SELECTION REQUIREMENTS:

. D( d; F" @$ K2 t# [Qualifications/Experience
( } N) j4 c3 s• Baccalaureate degree in Nursing preferred* }/ U5 K, D( u J) T, S) [
• Preference given to those with at least 5 years nursing experience in primary care setting.4 |% g# J6 c9 ?6 X7 c3 U5 L
• Current CARNA Registration
3 N9 }8 @1 K' r/ } z) k7 v• Current CPR – Healthcare Provider- |6 ~7 f' s. |) w
• Must have own vehicle and valid driver’s license
