; x. _8 B8 r; z" fThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

) P2 T. `* m' v( K- aThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.. H9 E5 S4 c0 {5 l1 a

The Nurse will report to the Clinic Coordinator.

1 h1 c' j! }# c/ p0 l" H1 P•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.1 @) ?6 T4 p8 s
•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
* u* c4 R% j+ b4 U•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.8 k& ]  B; u) Y3 t1 L" c
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

Clinical Role

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  / X6 E0 d4 U) ~2 f# {4 ?, ^2 M
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. * d! L) a4 w6 F8 u, Z( ~0 d8 |
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.$ |3 _3 Y" K; a9 r
•        Ensure the maintenance of accurate records in line with practice policy.
0 ?8 J) F/ |, v, j8 P5 [•        To assist in developing and implementing patient self-management programs.
1 G6 C1 V- ]+ p6 R•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.% f* P3 T  E! t+ a7 ]
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.
; l, J7 T" F4 I" j) c8 O•        To offer training support for other team members and assist in their professional development.

•        He/she should be able to work in a team while respecting the views/opinions of others.
% U3 i# g+ d/ X  f  p) J2 g. J•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.* O9 k2 \# I& c1 }4 {7 l+ S7 {
•        Requires the ability to work independently and prioritize workload.
6 N( H, z+ }1 D: F•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.
+ z: Z: m8 w9 }. j  r•        To participate in the Network’s education program.

' ]& D' J% a! G! A, q) @& E
9 x+ t+ {+ D' B# `) l! R  J- zQualifications/Experience
+ a& ?& _& V1 D& J1 [. R- |, v•        Baccalaureate degree in Nursing preferred
+ z, N7 j: f3 v•        Preference given to those with at least 5 years nursing experience in primary care setting.
3 o$ A+ K4 s; `* T- {- a. j! n; \•        Current CARNA Registration3 m7 r. X, C6 s3 m4 U
•        Current CPR – Healthcare Provider* W) A: E5 M. F' [
•        Must have own vehicle and valid driver’s license* b% k: Q$ F8 b( E" y2 o( H
•        Knowledge of, and ability to use, electronic medical records' Y' X: b* J8 H# F+ }  n9 Q) y; P
