The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population., L9 y/ ^1 I* Q6 F' c

• To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.3 J) S5 e+ V I- O
• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To assist in the implementation of strategies for improving the quality and provision of care for the practice population.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.

• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.! V, g. y6 ^3 @: r: s
• Ensure the maintenance of accurate records in line with practice policy.* N' _! x: G1 @, f( r+ H& Y( t
• To assist in developing and implementing patient self-management programs.

• To offer training support for other team members and assist in their professional development.

. V% j* a9 R$ C, E9 T5 d- g% l• Baccalaureate degree in Nursing preferred6 h) A4 l1 P% o7 N; A/ D* F
• Preference given to those with at least 5 years nursing experience in primary care setting.7 O% v2 V; N& h$ A$ j0 N
• Current CARNA Registration" G' i* q' G/ N; }8 B
• Current CPR – Healthcare Provider p- c5 _8 ~; P4 C: z c% Y
• Must have own vehicle and valid driver’s license
6 Q4 }9 g) ?2 M! V9 C1 U3 b• Knowledge of, and ability to use, electronic medical records
