TThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.; I3 F5 F! w' t3 l! The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.% o# ?; N$ E! H9 ~# k

! ?3 r# w7 S1 \% q9 Z5 w•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
" ~1 T; J" ~+ ^* Q- a! x•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.. L% s9 R# @* Z- k( `/ c
•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.' B3 g; I* C; Z- F( N* f% }1 d$ x
•        To assist in the implementation of strategies for improving the quality and provision of care for the practice population.( s3 g1 i7 b% ]: O  O; J
•        Educator, communicator, advocate, coordination/navigation, supporter

Clinical Role

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  
0 g9 r" J/ Q& s! g8 V& i•        To develop long-term, trusting relationships with both patients and peers in providing primary care services. + z0 u6 N+ @& y7 Z0 l/ R0 c! [
•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.
5 @9 l! d/ v% s2 r% v•        Ensure the maintenance of accurate records in line with practice policy.- W- A8 U) h: ^. U$ R5 j/ U& P
•        To assist in developing and implementing patient self-management programs.& @/ q3 i; h$ x0 U
•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.3 _9 S3 x2 `" Z0 t
•        To act as a resource to other multidisciplinary team members, physicians, and community resources.

•        The primary care nurse should have excellent written and verbal communication skills.& T  x  W, d. g. k; ]
•        He/she should be able to work in a team while respecting the views/opinions of others.
- {% j/ ]7 |' G5 k; Z3 c4 w•        Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.* h% Q+ B4 w* V! ]
•        Requires the ability to work independently and prioritize workload.& d8 l3 \- C6 Q; D
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.

- p6 K2 C1 k( x# z7 YSELECTION REQUIREMENTS:9 J' L0 ~* o4 [* ?/ Y
, [; s& M* x+ r; \. m" N5 i
Qualifications/Experience
4 \/ ]4 d2 g) `( E" j•        Baccalaureate degree in Nursing preferred, x0 G/ R  _5 P% p" ]3 N
•        Preference given to those with at least 5 years nursing experience in primary care setting.
* Z* y' {- D$ q( y- B2 H% z- F& r•        Current CARNA Registration
9 b2 r* ~# K; F4 Z0 s  f5 C1 ?7 l•        Current CPR – Healthcare Provider6 n! l( P% ^) }+ n! B& n* e
•        Must have own vehicle and valid driver’s license
$ D. @. O$ c, U& Z& E) d6 \•        Knowledge of, and ability to use, electronic medical records
