The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

Clinical Role

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. * J) U# f$ V1 X5 n1 R$ P
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. ; Q+ Z; V+ }( w) Q7 B2 R
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.9 h. T: T+ F6 B5 [1 I$ n$ C; o# k# J
• Ensure the maintenance of accurate records in line with practice policy.1 p; H# C: e3 K% @0 |6 k
• To assist in developing and implementing patient self-management programs.: a$ i/ j' a5 C! v S2 k6 R8 v' q
• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

Qualifications/Experience
2 x+ b8 T6 N* L+ ?6 g• Baccalaureate degree in Nursing preferred- o2 R$ S) Y5 P w: k5 ?
• Preference given to those with at least 5 years nursing experience in primary care setting.
?9 n5 Q* K3 T7 [6 |0 P: s• Current CARNA Registration( q& O4 g. ^( W
• Current CPR – Healthcare Provider3 W, x/ Y F( ]) X+ ?' N
• Must have own vehicle and valid driver’s license# ^" J) I* C6 R/ d1 m
• Knowledge of, and ability to use, electronic medical records0 `# E, @/ z) M. L$ U( c
