Any document that is not in English or French must be accompanied by

•the English or French translation, and
$ R* a& N( {2 a" g•an affidavit from the person who completed the translation.
Note: An affidavit is a document on which the translator has sworn, in the presence of a person authorized to administer oaths in the country in which the translator is living, that the contents of their translation are a true translation and representation of the contents of the original document. Translations by family members are not acceptable.Family member is defined as being a: parent, guardian, sibling, spouse, grandparent, child, aunt, uncle, niece, nephew and first cousin.

哪位大侠给解释一下，俺随便找个人就可以做还是必须到专门的机构去做哇，还有这个affidavit有专门的固定格式吗？

不是谁都可以做。记得在INSTRUCTION里专门有一页列出是谁可以做这个。

我以前做过一个类似的。当时找一个机构做的，由它的一名志愿者帮忙翻译，之后在下面有个证人的签字和盖章，是一位COMMISSIONER FOR OATHS的。这样就可以了。
