内容上除了"BENEFIT"没有写明是什么福利,其他倒是都全了.最最重要的是它的LETTERHEAD, 签字人的头衔和联系方式.用正式的公司信签. 最好出信的日期晚一点, 你的都是半年多前的了. 留原件,交复印件. 想要更保险,连同雇佣合同/工资单一起递交.

我以前是自己写的，然后让雇主签字。雇主也省事，自己也可以按要求写。
