Target:        Low income families with children/youth aged 3-17 residing in Edmonton

Time        9:30am – 11:30am
4 N; j+ m0 M; A& H# m- O! `2 c! CPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
. }7 g) e" ?) a1 p# U, EFees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
6 W  }1 G- c3 b+ ]" F2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
! [3 P  K! {8 S- ?4 ]' n0 r  t5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

+ n. A- T  J+ ?1 a: c" O" x9 @8 iWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
