7 n4 Z2 G" ^0 i& Y0 s. i& Y: _Date        May 14, 2005 (Saturday)
0 k9 H. N$ g$ Y. H5 V. DTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

Remarks:
! _# A  q% n5 H9 G% f) e# m1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
. N  @! Q" T$ }! p3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
+ G8 F$ {# N3 r5. Children are not required to be present at the Registration Day
) ~% N4 J; z' j+ M2 v6 a2 _6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
