provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services.
1 q) H' I7 @, X+ F" oThe position builds working relationships with New Canadian Health Network partners’ staff, * n2 S: k2 z" v2 O
newcomers accessing these services and other institutions and service providers. The Social
5 l K D) O p3 MWorker will assist in case planning, conferencing and providing support and advocacy as ( k8 ?3 B" A" o9 m k
needed. As required, the Social Worker will assist in accessing community supports for
, {4 X w; _1 Y1 snewcomers and their families. The Social Worker provides holistic and flexible initial 5 o/ E3 {; C2 ]& ^* v
assessment of life circumstances, needs, goals and aspirations and adheres to the client-! @: Z. g8 B( ~/ k- _1 Q& q
centered delivery model. ; \8 n/ d2 d2 R* X3 e

case plan support for clients who have complex needs.

Relevant undergraduate degree or diploma, or equivalent combination of education and
0 I2 X7 m% ?8 ]2 Texperience in Social Work or related human service fields.
- @6 m& c& @6 g: i- l' G Extensive knowledge of community resources, including in depth understanding of family
. b7 c8 G |' Z1 U7 Bsupport needs

Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
