Nature of Work: The Social Worker will report to the manager of mental health services.
& [% L1 c5 P& L+ M" Z3 o& S$ eThe position builds working relationships with New Canadian Health Network partners’ staff, ! o6 x0 U7 Y% m; x7 _4 K& ~" c
newcomers accessing these services and other institutions and service providers. The Social
' s( J) a) R2 Z( }6 v) T# W$ bWorker will assist in case planning, conferencing and providing support and advocacy as
( V G/ U( M9 {; m2 s$ fneeded. As required, the Social Worker will assist in accessing community supports for
7 v1 D$ ^" m% R! ]5 Znewcomers and their families. The Social Worker provides holistic and flexible initial ; c$ b0 A7 ]1 D+ `% E
assessment of life circumstances, needs, goals and aspirations and adheres to the client-; b' N5 }/ b; C. z: @8 N
centered delivery model. 0 W$ P4 a2 X2 W; X

Developing of social networks and community supports for newcomer families. 4 T$ f5 Q! i2 L6 x+ S7 A0 T
Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental

Advocate on newcomers’ behalf to programs, institutions and service providers.

; g. o P3 q3 y8 ]+ H Relevant undergraduate degree or diploma, or equivalent combination of education and
; B( k- ]8 `% Q( ?7 b5 P& hexperience in Social Work or related human service fields.
' r6 {% A% \7 G Extensive knowledge of community resources, including in depth understanding of family
: y0 D' P0 y# f |8 b0 Z4 vsupport needs
9 b" R9 V" I0 f) O. h Experience/facility with MS Office computer applications : I- o9 w2 [: c. m$ c
Proficiency in both official languages is an asset, as is fluency in other languages . Y9 `+ X$ p. z" d! r7 f$ b( v5 u
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate $ y; r, @* j, U

Experience: Minimum of 3 years experience working in a related field - S& a/ }3 ] M/ S
