Primary purpose: The Social Worker works in partnership with settlement workers or 2 T3 H: _+ D1 z' s p X: W

provide supplemental support to newcomers who have complex needs.

$ y9 |& I) _5 I7 z/ s/ x Nature of Work: The Social Worker will report to the manager of mental health services.
% Y! C m& v+ Z, {9 p0 m4 K. p3 O. JThe position builds working relationships with New Canadian Health Network partners’ staff, ! r A, \! V1 @+ L3 ], T
newcomers accessing these services and other institutions and service providers. The Social $ \" a8 h, U: J" B* {7 R, N: \
Worker will assist in case planning, conferencing and providing support and advocacy as 6 H6 S _. Z' d/ F! t# o) k
needed. As required, the Social Worker will assist in accessing community supports for 3 |. M' z6 F( _3 P- }9 C K
newcomers and their families. The Social Worker provides holistic and flexible initial

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental

Advocate on newcomers’ behalf to programs, institutions and service providers.

Relevant undergraduate degree or diploma, or equivalent combination of education and
! F& A3 v0 }2 H: bexperience in Social Work or related human service fields.

support needs

Experience: Minimum of 3 years experience working in a related field
