PRIMARY CARE NURSE

$ U) C# g4 z) \4 |0 t1 m9 ?! ^& bThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network.  The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient.  The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.

& K; C, Y0 o6 L$ ^The Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

•        To work collaboratively within a family practice environment to enhance the delivery of primary care services.
•        To receive patients and provide individualized health care and services based on highly developed nursing knowledge and skills.
* Y$ M1 W3 N( R  s1 O•        According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.
  X2 J! B3 W, O; x; \•        To make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners.

•        To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.  
•        To develop long-term, trusting relationships with both patients and peers in providing primary care services.
: ?2 X3 }% j" x" p& p•        To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.
•        Ensure the maintenance of accurate records in line with practice policy.
" i+ x6 W( V# S2 X! ^8 z•        To assist in developing and implementing patient self-management programs.
•        To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.

•        To offer training support for other team members and assist in their professional development.

•        The primary care nurse should have excellent written and verbal communication skills.
•        He/she should be able to work in a team while respecting the views/opinions of others.

•        Requires the ability to work independently and prioritize workload.
•        To be responsible for own professional development through a variety of strategies highlighted in personal development plan.

SELECTION REQUIREMENTS:
# I  Y% k0 ]0 }2 C- X& W: u) H
* T, P3 d, m  O& x# O: x7 UQualifications/Experience
7 v/ \" d" g5 W! |/ Z6 G# L•        Baccalaureate degree in Nursing preferred
# ~6 g, E1 q' S! y- q- t•        Preference given to those with at least 5 years nursing experience in primary care setting.
•        Current CARNA Registration
- Y% l4 [8 D( H2 ^& Q: I•        Current CPR – Healthcare Provider
, [  T5 P2 }  ~( ^4 T•        Must have own vehicle and valid driver’s license
