5 j- q/ M7 q% t+ ]* N( _The Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.$ D6 H! x0 c+ W. {6 c2 ?- b# o; o
/ e' e- P( Y+ T; xThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. / m/ w. ^. o7 \% R" s+ W. a/ U
• To develop long-term, trusting relationships with both patients and peers in providing primary care services.

Qualifications/Experience
3 X- Y+ K- t: o9 h• Baccalaureate degree in Nursing preferred6 {3 I0 ^- A. \+ x/ _5 e9 B/ G
• Preference given to those with at least 5 years nursing experience in primary care setting.
/ A( M/ X* Z* }( r Q* p2 H• Current CARNA Registration3 V. R" H6 L9 O1 ~, ~8 T7 W0 A
• Current CPR – Healthcare Provider. z4 v% Q3 s# S8 Y# I) j# v
• Must have own vehicle and valid driver’s license1 F' r, p* |7 t( n) x" e: n
• Knowledge of, and ability to use, electronic medical records
