1.Employer’s account number----是雇主的帐号吗？具体是指什么帐号？
2.Name of the tax services office that processed a tax waiver for this credit
; C* R( e- T/ c4 _" ](if one was requested)----是什么意思？
3.I am an authorized signing officer of the business-----这里谁可以签字？
