When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic1 I( O Z- n; ?: ^& M1 J
condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and & s$ Y& [2 F" X3 N• there have been no new symptoms or change in symptoms; and/or @& q& d3 g1 ?- K' w. g7 ]2 c
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. 0 [1 C" r: |9 N$ m% z- GIf “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
