We encourage you to make every effort to resolve your concern directly with the dentist involved before making a complaint to the Alberta Dental Association and College. Do not delay treatment because you are waiting for the outcome of a complaint.& A2 i5 h/ ?$ ]$ K) |1 ~

If you do make a complaint to the Alberta Dental Association and College about the conduct of a dentist or a quality of care concern, your complaint must:

& x: G. Z7 G1 E9 |be signed by you;, y% h! ?# x% w1 Q
2 P: ^2 c: k9 `- x: z3 _provide the name of the dentist you are complaining about;

include enough detail about the nature of your complaint so the dentist is able to respond to the complaint, such as what happened, when it happened and where it happened;9 g0 T9 {; B1 d) ?, g

list your contact information including your mailing address and phone number; and
