•At any City of Edmonton leisure, sports & fitness participating facilities.
•14th floor CN Tower, 10004-104 Ave.
! ?0 h. _' r1 x) I6 p2 R7 c+ r) D: s Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
. A: c7 S* B5 k% K- q( P3 e•For individuals under custodial care, please also submit the address verification form.
